Common use

Orap (pimozide) is prescribed for the treatment of severe motor and vocal tics in patients with Tourette’s disorder when other treatments have not been effective. It works by blocking dopamine receptors in the brain, helping to reduce involuntary movements and vocalizations.

Dosage and direction

Orap is taken orally, usually once daily or as prescribed. The medication should be taken with water and can be taken with or without food. Dose adjustments must be made only by a healthcare provider, as the medication requires careful monitoring due to its effects on the heart and nervous system.

Precautions

Before taking Orap, inform your doctor if you have heart problems, a family history of sudden cardiac death, electrolyte imbalances, liver problems, or a history of seizures. Regular ECG monitoring may be required. Avoid alcohol and be cautious with activities that require alertness. Do not stop the medication abruptly without medical advice.

Contraindications

Do not use Orap if you are allergic to pimozide or similar medications. It is contraindicated in patients with known QT prolongation, arrhythmias, severe electrolyte imbalances, or those taking certain medications that affect heart rhythm. It should not be used with strong CYP3A4 inhibitors.

Possible side effect

Common side effects include drowsiness, dizziness, dry mouth, constipation, blurred vision, or restlessness. Serious side effects may include heart rhythm abnormalities, uncontrolled muscle movements, severe stiffness, tremors, or signs of neuroleptic malignant syndrome. Seek immediate medical attention if severe symptoms occur.

Drug interaction

Orap interacts with many medications, including antidepressants, antipsychotics, antibiotics, antifungals, antiarrhythmics, and drugs that affect electrolyte balance. Grapefruit products may also interact. Always inform your doctor about all medications and supplements you are taking.

Missed dose

If you miss a dose, take it as soon as you remember unless it is almost time for the next dose. Skip the missed dose if needed—do not double the dose.

Overdose

Overdose symptoms may include severe drowsiness, confusion, tremors, seizures, or dangerous heart rhythm changes. Seek emergency medical attention immediately.

Storage

Store Orap at room temperature, away from moisture, heat, and light. Keep the medication in its original container and out of reach of children and pets.
